- Authenticated Birth/Marriage Certificate from PSA (Formerly NSO) issued within one year
- Personal Appearance by Appointment
- Proof of Assets like Car Registration and Land Titles (if any)
- Minor Applicants: School registration/proof of enrollment
- PSA (Formerly NSO) Birth Certificate
- Affidavit of Support and Consent if not travelling with either or both parents
- Renewal (no need for appearance) US DROP BOX SERVICE
- Eligibility: US Visa still valid or Visa has NOT expired for more than 12 months
